Requesting Fraser's Banners

Because of an increase in demand and my time available to make banners being short, I'm putting in place a list of requirements that must be fulfilled before I can make the requester a banner. This is being put in place because I don't want to waste my time on a banner for it only to be used 5 times, I need to have some sort of reassurance that you'll be sticking around for awhile and you'll get some use out of it.

Requirements:
1. 50 Post Count
2. Been a member at the Message Boards for 1 month
3. Have a Pilot ID

If you've checked off all three of those, then send me a PM with these details:
1. Name
2. Pilot ID
3. Three preferred airlines (only airlines we operate a VA of)
4. Three preferred aircraft (only aircraft that fly for the 1 or more airlines you chose)
5. Management Positions (If applicable)
